HIGH SCHOOL APPLICATION FOR ADMISSION

CANDIDACY REQUIREMENTS FOR ACCEPTANCE:
An application is considered complete when all parts of the application packet are received at Desert Christian High School. After Admissions Committee review, qualified candidates will be contacted by mail or phone to schedule a family interview. A placement exam will be required (time and date to be announced). Families will be notified by mail of the final acceptance decision in early-April.
APPLICATION PROCESS:
1. Complete the Student Application (one per applicant) and attach a current school photograph.
2. Complete the Family Application (one per applicant).
3. Complete the Special Health / Learning Information form.
4. Submit completed forms with the non-refundable application fee of $75.00 to:
Desert Christian High School, 7525 E. Speedway Blvd. Tucson, AZ 85710, ATTN: Admissions
5. Fill in the top portion of the remaining applicant’s forms indicated below, sign, and give them (along with a school-addressed, stamped envelope) to the appropriate person as indicated on each form. These forms must be completed and mailed directly
*Church Life Recommendation
*English Teacher Recommendation (must be current teacher).
*Mathematics Teacher Recommendation (must be current teacher).
*Home School Recommendations (if home schooled).
Request applicant’s school to send an unofficial copy ofto the DCHS office (address above):
*Grades
*Standardized Test Scores
*Immunization Records
*High School Placement Test Scores (if previously taken)
ADMISSION AS STUDENT:
Following acceptance, an enrollment packet will be mailed to you.
Return the completed enrollment form to the school office, accompanied by the designated enrollment fee.
Families will be notified of and requested to attend the New Student Orientation evening in May.
